Evercare evaluation interim report: implications for supporting people with long term conditionsThis interim report is published by a team of researchers from the National Primary Care Research and Development Centre, Manchester Business School and the University of Sheffield, Institute for Studies into Ageing, who are conducting an independent national evaluation of the Evercare programme.



Evercare programme, which combines nurse-led care and case-management as a way of reducing the number of unplanned emergency admissions of frail elderly people, may not be as effective as first thought, according to research out today.



The Evercare programme is being piloted in 10 Primary Care Trusts in England, but the principles of case-management of elderly people are already being rolled out in the 2005 NHS policy “Supporting People with Long Term Conditions”, including the appointment of three thousand community matrons.
