The vast majority of NHS patient encounters occur in primary care healthcare settings. Data collected during those encounters sit in thousands of separate databases (mainly GP practice systems) around the country. This is a pity because the way in which primary care data are recorded means that much more detail about the health status of patients (observations, investigation findings, diagnoses, treatments and outcomes), is recorded than in any other healthcare setting. Furthermore the opportunities for capturing data means that in an aggregated form the data set should be able to give as complete a picture about the health of the national population as it is conceivably possible to get.
